Multi-source feedback surveys

Multi-Source Feedback (MSF)

Our multi-source feedback tools allow individual practitioners to obtain unique and varied feedback from peers, patients and industry associates to gain more significant insights into their all-around performance. Our MSF offerings are approved by a number of industry and regulation bodies for the award of continuous professional development (CPD).

Interpersonal Skills Questionnaire (ISQ)

Designed to give health professionals insights into their interpersonal skills from the patients’ point of view, our Interpersonal Skills Questionnaire (ISQ) addresses recent care experience and interactions with individual practitioners or surgeons. Clinicians can gain insights into the humanistic aspects of their patients care experience to better understand aspects of their own professionalism.

Colleague Feedback Evalutation Tool (CFET)

Our Colleague Feedback Evaluation Tool (CFET) is designed to give GPs, specialists and surgeons insights into their professionalism and behaviours from the point of view of their peers and colleagues. It supports clinicians to better understand elements of their working relationships and professional competence as perceived by their peers. It asks, ‘Do I see myself as others see me?’.

Patient reported measures (PREMs & PROMs)

Patient reported measures is an umbrella term covering Patient Reported Experience Measures (PREMs) and Patient Reported Outcome Measures (PROMs), including the Patient Activation Measure® (PAM®). PREMs and PROMs tools can be used alone or together to assist in continuous quality improvement initiatives.

PREMs are used to measure patient experience. These questionnaire-based experience measures capture a patient’s experience of care. This data is particularly valuable as it can identify opportunities for quality improvements at the service level while also informing government policy via key trends to support system-wide change.

PROMs capture information about a patient’s quality of life or condition-specific measures. For example, how diabetes is impacting their life. Responses are directly reported and capture the patients’ perspective about how illness or care impacts their health and wellbeing.

Our Practice Accreditation and Improvement Survey (PAIS) is a patient feedback solution specifically designed to meet the quality improvement needs of general practices. As an approved tool by the RACGP, our PAIS offering supports accreditation requirements as part of the RACGP Standards 5th edition.

Active Insights is an agile digital feedback solution that provides organisations with real-time patient experience data and insights.

Collecting and collating qualitative and quantitative feedback data and presenting it in live, interactive dashboards, Active Insights helps you convert patient feedback into better patient experiences: better care and service delivery, more efficient  practice management and improved business outcomes.

Our Patient Partnership in Care (PPiC) tool is a validated, evidence-based feedback tool developed to assess Patient Reported Experience Measures (PREMs) and Patient Reported Outcome Measures (PROMs) in patients with chronic disease. The PPiC tool can further support care teams to understand their strengths and how they might improve their partnerships with patients.

Our Diagnostic Imaging Survey is an essential tool in highlighting consumer perceptions around a diagnostic imaging services’ service and the delivery, safety, quality and effectiveness of care. Healthcare teams can use the insights delivered by the survey as reliable evidence to meet the requirements of the Diagnostic Imaging Accreditation Scheme (DIAS).

Our Pharmacy Questionnaire plays an integral role in supporting pharmacies to improve the quality of their services and care. It provides pharmacies with insights into person-centred approaches and allows them to identify strengths and opportunities for development.

Our Clinic Improvement Survey (CIS) is a patient feedback solution specifically designed to meet the quality improvement needs of specialist practices, and allied health providers. Findings from our CIS tool can be used as reliable evidence to support relevant accreditation requirements of the NSQHS Standards and the Primary and Community Healthcare Standards (PHCS).

Our Dental Practice Questionnaire is an integral tool in supporting a dental practice to improve the quality and safety of services and care provided. Report findings can also be used as reliable evidence to support relevant accreditation requirements of the NSQHS Standards (first edition) or the Primary and Community Healthcare Standards (PHCS).

Our Retirement Village Resident Feedback tool supports retirement villages in improving the quality and safety of its services, environment, and support to enhance the resident experience. Report findings can also be used as reliable evidence to support relevant accreditation requirements against the Australian Retirement Village Accreditation Scheme (ARVAS).

Patient Activation Measure® (PAM®)

Patient Activation Measure® (PAM®) is a world-renowned assessment tool that has been adopted by leading health systems and organisations. CFEP Surveys holds the exclusive Australian license of PAM® to enable healthcare practitioners and services to improve risk identification, assist in tailoring individualised care plans, and evaluate a range of programs.
